Gracious Care Adult Foster Home offers personalized support and care from Level 1 to Level 3 within a nurturing, home-like environment. With a capacity for only five residents, we ensure a minimum of two caregivers are present during the day and one awake caregiver at night, providing compassionate assistance for individuals with Dementia, Alzheimer’s, and those requiring Hospice and Palliative Care around the clock. In addition to our dedicated care staff, Gracious Care AFH facilitates RN visits, Music Therapy, and Walking Companions for residents who wish to explore the nearby nature park or need additional support.

 

Located in a serene area at the heart of Cooper Mountain, close to Cooper Mountain Nature Park, our facility features comfortable living spaces, secure outdoor areas, home-cooked meals, and a range of daily activities—all designed to promote comfort, dignity, and meaningful relationships.
